A 48-year-old woman developed acute megakaryoblastic leukemia. This patient was
presented with anemia and a low percentage of blasts in the peripheral blood. Her
marrow showed marked reticulin fibrosis with an increase in blasts and immature
megakaryocytes. Rarely Auer rods were noted in the blasts of peripheral blood.
However, it was confirmed that the majority of blasts were of megakaryocytic origin by
the indirect immunofluorescent test with monoclonal antibody against platelet membrane
glycoprotein Ⅲa and by ultrastructural analysis of platelet peroxidase. Cytogenetic study
showed several chromosomal abnormalities, 45, XX, -20, t(7 ; 10) (22q ; 22q).
It is considered that there is a mixed population of megakaryoblasts and myeloblasts
in this case. However, a predominant megakaryoblastic component is consistent with a
diagnosis of acute megakaryoblastic leukemia, M7.
